Ready to enroll your child in Catholic Charities Early Learning & Head Start or Early Head Start? Download and complete the forms below.

Your child will also need an updated physical and dental exam. Please fill out the appropriate forms, and bring them with you to your appointment. If you do not have these documents, please let enrollment staff know so they can provide you with the next steps.

We also ask all parents who enroll their child in any of our Early Learning & Head Start or Early Head Start programs to please read our parent handbook and review our school year calendar.
