Employment & Training Services at St. Philip Neri Family Center is pleased to present its Ohio Board of Nursing certified Community Health Worker training with a focus on reducing infant mortality.
In partnership with the Institute of Community Health, we have developed an accessible, comprehensive training program that incorporates adult learning methods and meets the needs of diverse learners.
Community health workers provide an important link to improve the health and well-being of individuals in their own neighborhood. They are members of the communities they serve and are front line public health workers. They are essential to improving quality of care and breaking down cultural barriers to treatment.
The fall training session begins on September 15, 2020, and ends November 30, 2020.
This 12-week program is open to applicants who have a high school diploma or equivalent AND a desire to serve others in their community. Students are required to complete 100 classroom hours and 130 practicum hours to graduate from this program.
This program is free for individuals whose income at or below 200% poverty, and/or receive FCDJFS Benefits (Food Assistance, Medicaid, Child Care, Ohio Works First).
If interested in applying for the Community Health Worker program, please complete the application below and send verification of High School Diploma or Equivalent to awinfield@ccdocle.org
